Athletic Clearance<\/strong><\/p>\n

At Monterey Trail we use an online clearance process to clear our athletes. The website is <\/span><\/span>www.athleticclearance.com <\/strong><\/span><\/a><\/span><\/p>\n

ATHLETIC ELIGIBILITY<\/strong><\/p>\n

Every student who wants to participate in a sport must complete and have signed by a parent\/guardian the following forms: 1. Physical \/ Medical Insurance (Mandatory) 2. Emergency Information (Mandatory) 3. Agreement for Team Participation (Mandatory) 4. Student Alternate Transportation Form (Mandatory) 5. Sudden Cardiac Arrest Form (Mandatory) 6. Concussion and Head Injury Information Sheet (Mandatory) 7. Student Personal Automobile Use Form (Optional) Athletes must have a yearly physical from a qualified physician who completes the medical examination report. In\u00a0order to be accepted for athletics, the physical must be completed after May 1, 2020 of the school year in which the athlete plans to compete.\u00a0Completed papers should be given to the head coach\/athletic director depending on the site\u2019s athletic department policy.<\/strong><\/p>\n

Students will need to create an account and upload a current physical after May 1<\/span>st<\/span>, 2021 for the School year of 2021-2022. Also you can find the EGUSD physical form on the <\/span><\/span>mthsmustangs.com<\/a> <\/strong><\/span>under the Forms page. If you have any questions about clearance. You must be academically eligible from final term\u00a04 grades to tryout for a\u00a0fall sport.\u00a0<\/span><\/span><\/p>\n
